Start with the person, the goals, and the support needed to succeed.

Referral review considers participant goals, support needs, health and safety, transportation, staffing, authorized services, and compatibility with Empower 365’s ambulatory-only community-based model.

Referral information

Participant name, age, contact information, and preferred communication method.
Regional Center or funding source and service authorization information when available.
Current IPP goals, desired outcomes, strengths, interests, and support needs.
Relevant health, behavioral, safety, communication, mobility, or supervision information needed to evaluate fit.
Transportation needs, schedule considerations, emergency contacts, and any applicable support plans.

What Happens Next

Referral review is individualized.

Following document review, Empower 365 may coordinate an intake meeting and program tour, assess compatibility, identify additional information needed, and communicate next steps with the participant, planning team, and Regional Center as appropriate.
