Who does Empower 365 serve?
Empower 365 is designed to serve adults with intellectual and developmental disabilities through an ambulatory-only, community-based service model.
What does “community-based” mean?
Participants use community settings as part of learning and skill development. Activities may involve transit, businesses, libraries, parks, volunteer sites, employment-related settings, recreation, civic resources, and other inclusive environments.
Are services connected to the participant’s IPP?
Yes. Service delivery and documentation should connect to authorized goals, participant preferences, desired outcomes, and current planning information.
Does Empower 365 provide employment preparation?
Employment-related support may include vocational exploration, résumé development, interview preparation, workplace behavior, work habits, skill identification, and preparation for Competitive Integrated Employment when consistent with the participant’s goals and authorized service.
How do I make a referral?
Start by calling or emailing Empower 365 or using the contact page. Referral information typically includes the IPP, goals, support needs, safety and health information, transportation needs, funding information, and other documents needed to determine compatibility.
Does submitting a referral guarantee admission?
No. Admission depends on program fit, current capacity, authorized services, safety, staffing, transportation, and whether the program can appropriately meet the participant’s needs.
