Empower 365 Community Services provides person-centered services for adults with intellectual and developmental disabilities. We connect individual goals to practical learning, community participation, employment preparation, travel skills, self-advocacy, and greater independence.

The community is a learning environment. Participants practice skills in real settings while receiving individualized coaching connected to their goals, preferences, and support needs.
Community Integration
Volunteerism, recreation, cultural experiences, civic participation, social connection, community exploration, and development of natural supports.
Independent Living Skills
Budgeting, shopping, meal planning, communication, personal routines, household management, organization, and informed decision-making.
Employment Preparation
Vocational exploration, résumé and interview preparation, workplace behavior, work habits, career readiness, and pathways toward Competitive Integrated Employment.
Travel Training
Route planning, public transit, pedestrian safety, trip preparation, community navigation, and progressive travel independence.
Technology Literacy
Smartphone and computer skills, digital communication, calendars, navigation tools, online safety, and practical technology use.
Self-Advocacy & Wellness
Choice-making, self-determination, relationships, boundaries, emotional regulation, healthy routines, and confidence in speaking up.
Person-centered support must be visible in daily practice.
Participants are adults and active partners in service planning. Staff are expected to teach, coach, prompt, model, and fade support when appropriate so people can increase their own participation and capability.

Empower 365 is designed as an ambulatory-only, community-based service model. Referral decisions consider individual goals, support needs, safety, staffing, transportation, authorized services, and whether the program can appropriately meet the person’s needs.
